1. Photographic I.D. (preferably a passport). A photocopy of this can be sent, but the real thing must be presented before you are assessed.
2. A deposit of £150,

4. Four colour passport-type photographs of yourself*. .

Making an appointment for an assessment
Before your first appointment at the Clinic, you will need to have completed a Personal History Form. You can do this either by printing out and sending a PDF by post, or by completing a Microsoft Word document and sending it by email.
PDF (PDF requires printing and filling in by hand.
This can then be sent by post to the clinic address: THE STAPLEFORD CENTRE, 25A ECCLESTON STREET, LONDON, SW1W 9NP
Microsoft Word (This format can be filled in and saved to your computer, then emailed as an attachment to info@staplefordcentre.com)
